RESPONSIBILITIES
- Provide compassionate psychological support to elderly patients in Assisting Living Communities, Skilled Nursing Facilities and Long-term Care facilities and independent patients via Telehealth.
- Conduct assessments and counseling sessions in person or remotely via Telehealth, depending on patient needs.
- Establish open lines of communication for individuals with mental or emotional issues.
- Aid individuals with various mental health issues.
- Implement and review treatment plans.
- Collaborate with additional treatment providers and the client’s care team, when needed.
- Ensure all documentation is completed in a timely manner.
- Document patient interactions and progress with care and accuracy.
- Collaborate with facility staff and medical teams to ensure holistic patient care.
- Maintain professional boundaries and ethical standards in all interactions.
- Be responsive — when a patient needs you, you show up. Excuses aren’t in your vocabulary.
REQUIREMENTS
- Licensed Psychotherapists including Psychologists, LCSWs, LMHCs/LPCs/M&FTs
- Excellent communication and listening skills.
- Compassionate, reliable, and highly organized.
- Ability to work independently and manage a flexible schedule.
- Prior experience with geriatric or hospital-based populations is a plus, but not required.
